Subject:                      Resolution authorizing a Master Service Agreement with Radiology Associates of Central Florida, P.L.

Staff Recommendation:

Staff recommends approval of a resolution authorizing a Master Service Agreement with Radiology Associates of Central Florida, P.L. (dba Lake Medical Imaging).

Analysis:

The Master Service Agreement provides the general terms and conditions under which the customer will place orders for services from the City’s Communications Utility.  The customer has an immediate desire to order a connection between a main office in Leesburg and a remote office in Mount Dora.  This company has a strong preference for communications services from the City and will very likely order other services in the future.

Options:

1.  Approve the resolution, or

2.  Such alternative action as the Commission may deem appropriate

Fiscal Impact: 

This agreement will result in the customer’s placing an immediate order for services whose revenues during the current fiscal year will total approximately $3,500 and approximately $5,000 in each future fiscal year.

A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF LEESBURG, FLORIDA, AUTHORIZING THE MAYOR AND CITY CLERK TO EXECUTE A MASTER SERVICE AGREEMENT WITH RADIOLOGY ASSOCIATES OF CENTRAL FLORIDA, P.L.; AND PROVIDING AN EFFECTIVE DATE.

BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF LEESBURG, FLORIDA:

            THAT the Mayor and City Clerk are hereby authorized to execute a Master Service Agreement  with RADIOLOGY ASSOCIATES OF CENTRAL FLORIDA, P.L. whose address is 801 East Dixie Avenue, Suite 104, Leesburg, Florida.

 THAT this resolution shall become effective immediately.

PASSED AND ADOPTED by the City Commission of the City of Leesburg, Florida, at a regular meeting held the 8th day of November 2010.
