Subject: Resolution authorizing acceptance of a matching grant from the Florida Department of Health to purchase a LifePak 15 Monitor / Defibrillator not to exceed the cost of $30,737.33.

Staff Recommendation:

Staff recommends acceptance of the Florida Department of Health  grant to  purchase a

LifePak 15 Monitor / Defibrillator.

Analysis:

The Fire Department currently has four LifePak 12 monitor/defibrillators.  They are   all

over ten years old and near the end of their functional life.  This grant will allow the Fire

Department to replace one of these machines.

Options:

1.  Accept the Florida Department of Health grant, or;

2.  Such alternative action as the Commission may deem appropriate

Fiscal Impact: 

The total cost is $30,737.33.  The grant  is a 75% and 25% matching grant.    The   Florida

Department of Health will provide  $23,053 and the City of   Leesburg will be  responsible

for $7,684.33.  Funds are available in the FY 2010 – 2011 budget for the match.  A budget

adjustment will be necessary to add the additional revenue and expense into the current

budget.

RESOLUTION OF THE CITY COMMISSION OF THE CITY OF LEESBURG, FLORIDA AUTHORIZING ACCEPTANCE OF A MATCHING GRANT FROM THE FLORIDA DEPARTMENT OF HEALTH TO PURCHASE A LIFEPAK 15 MONITOR / DEFIBRILLATOR OF $23,053.00 AND A CITY OF LEESBURG MATCH ON $7,684.33, NOT TO EXCEED $30,737.33; AND PROVIDING AN EFFECTIVE DATE.   

BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF LEESBURG, FLORIDA:

            THAT the Fire Department is authorized to accept a grant from the Florida Department of Health whose address is BEMS Grant Program, 4052 Bald Cypress Way, Bin C18, Tallahassee, Florida 32399-1738, for a 75% and 25% matching grant.

 THAT this resolution shall become effective immediately.
